What's the meaning of "There you go" and "here you are"?

Question:
When I go to anywhere such as restaurant , shopping mall ,etc, I've heard people say, " there you go " "here you are " "here it is ."
Can anyone explain the correct meaning of these phrase and when do I use them ?

It's just the expression when you want to hand or show something to someone.
There you go when the item is far away
Here it is when the item is closed.
