พิธีพระราชทานปริญญาบัตรในเมืองไทย เกิดขึ้นครั้งแรกเมื่อวันที่ 25 ตุลาคม พ.ศ. 2473 (83 ปีที่แล้ว) โดยพระบาทสมเด็จพระปกเกล้าเจ้าอยู่หัว และ สมเด็จพระนางเจ้ารำไพพรรณี พระบรมราชินี เสด็จพระราชทานปริญญาบัตรแก่เวชบัณฑิต (แพทยศาสตรบัณฑิต) ของจุฬาลงกรณ์มหาวิทยาลัย ณ ตึกบัญชาการ (อาคารมหาจุฬาลงกรณ์ ในปัจจุบัน) ในครั้งนั้น จุฬาฯได้ส่งหนังสือเชิญหน่วยงานต่าง ๆ 11 แห่ง เช่น กรมยุทธศึกษาทหารบก กรมยุทธศึกษาทหารเรือ สภากาชาดสยาม วชิรพยาบาล มหามงกุฎราชวิทยาลัย ราชบัณฑิตยสภา และโรงเรียนกฎหมาย เป็นต้น ตลอดจนพระราชวงศ์ เสนาบดี และข้าราชการชั้นผู้ใหญ่ มาร่วมพิธีด้วย
ในงานนั้น มหาอำมาตย์เอก พระวรวงศ์เธอกรมหมื่นพิทยลาภพฤฒิยากร ทูลเกล้าฯ ถวายฉลองพระองค์ครุยบัณฑิตพิเศษ แด่พระบาทสมเด็จพระปกเกล้าเจ้าอยู่หัว จากนั้น พระบาทสมเด็จพระปกเกล้าเจ้าอยู่หัวพระราชทานครุยกิตติมศักดิ์บัณฑิตชั้นโท (หรือมหาบัณฑิตในปัจจุบัน) ทางวิทยาศาสตร์แก่พระยาภรตราชา ผู้บัญชาการจุฬาลงกรณ์มหาวิทยาลัยในขณะนั้น และบัณฑิตชั้นเอก (หรือดุษฎีบัณฑิตในปัจจุบัน) แก่ศาสตราจารย์ นายแพทย์ เอ จี เอลลิส คณบดีคณะแพทยศาสตร์ในขณะนั้น แล้วพระราชทานปริญญาบัตรแก่เวชบัณฑิตจำนวน 34 คน เสร็จแล้วพระราชทานพระบรมราโชวาท และเสด็จกลับ
เสร็จพิธี สภามหาวิทยาลัยได้เสนอให้กระทรวงธรรมการ กราบบังคมทูลขอพระบรมราชานุญาตว่า "มหาวิทยาลัยขอพระบรมราชานุญาตสงวนธรรมเนียมนี้ไว้ คือ ถือว่าการพระราชทานปริญญาบัตรเป็นการหน้าที่นั่ง หากเสด็จไม่ได้ ก็จะเป็นการถวายปฏิญญาต่อพระบรมฉายาสาทิสลักษณ์ และรับพระราชทานปริญญาบัตรจากผู้แทนพระองค์" เมื่อพระบาทสมเด็จพระปกเกล้าเจ้าอยู่หัวทรงทราบแล้ว ก็พระราชทานพระบรมราชานุญาตตามนั้น ธรรมเนียมนี้จึงสืบต่อมาจนถึงปัจจุบัน
พระบาทสมเด็จพระปรมินทรมหาภูมิพลอดุลยเดช เริ่มพระราชทานปริญญาบัตรแก่สถาบันอุดมศึกษาต่าง ๆ ของรัฐ ตั้งแต่ปี พ.ศ. 2493 เป็นต้นมา หนังสือพิมพ์คำนวณว่า หากเสด็จพระราชทานปริญญาบัตร 490 ครั้ง ประทับครั้งละราว 3 ชั่วโมง เท่ากับทรงยื่นพระหัตถ์พระราชทานใบปริญญาบัตร 470,000 ครั้ง จึงมีผู้กราบบังคมทูลขอพระราชทานให้ทรงลดการเสด็จลง โดยให้พระราชทานปริญญาบัตรเฉพาะในระดับปริญญาโทขึ้นไปเท่านั้น แต่มีพระราชกระแสว่า พระองค์ "เสียเวลายื่นปริญญาบัตรให้บัณฑิตคนละ 6-7 วินาทีนั้น แต่ผู้ได้รับนั้นมีความสุขเป็นปี ๆ เปรียบกันไม่ได้เลย" นอกจากนี้ ทรงเห็นว่า การพระราชทานปริญญาสำหรับผู้สำเร็จในระดับปริญญาตรีนั้นสำคัญ เพราะบางคนอาจไม่มีโอกาสศึกษาต่อในระดับปริญญาโทและปริญญาเอก ดังนั้น จึงตรัสว่า "จะพระราชทานปริญญาบัตรแก่บัณฑิตปริญญาตรีไป จนกว่าจะไม่มีแรง"
พระบาทสมเด็จพระเจ้าอยู่หัวภูมิพลอดุลยเดช เสด็จพระราชทานปริญญาบัตรด้วยพระองค์เอง จนถึงปี พ.ศ. 2540 เป็นเวลากว่า 50 ปีที่เสด็จเอง แต่ในสมัยหลัง ด้วยประชวรพระโรค จึงมักโปรดให้พระราชวงศ์เสด็จแทน ... ในการเสด็จแทนพระองค์นั้น พิธีจะประดับธงพระปรมาภิไธยย่อ ภปร. จะใช้เพลงสรรเสริญพระบารมีเมื่อผู้แทนพระองค์เสด็จมาถึง และพระแท่นหน้าที่ประทับจะมีพระครุฑพ่าห์ สัญลักษณ์พระมหากษัตริย์ เพื่อสื่อความหมายว่าเสมือนพระมหากษัตริย์เสด็จพระราชดำเนินมาด้วยพระองค์เอง
