memo - France visa from USA

These are the documents...I'm about to do mine through French Embassy. It will take only 1 week. I will apply by mail (I live in the US).

1. The application for a short-stay visa (multiple entries)
2. One 2x2 photograph
3. My passport.
4. Reservation for my air tickets.
5. Documentary evidence of socio-professional situation, including
5.1 The employment letter issued by my company
5.2 Most recent 3 pay slips
5.3 My 2004 W-2 (US tax information)
5.4 The copy of my 401K statement
6. Documentary evidence of accommodation in France, specifically the hotel reservation.
7. Documentary evidence of means of support for the duration of the stay, including
7.1 Bank statement (showing current balance on my checking and saving accounts)
7.2 Three of my major credit card statements.
8. Proof of my heatlh insurance that will cover worldwide for emergency medical situations as well as for urgent medical and hospitalization care.
9. The credit card authorization form for the visa fee.
