ปัญหากลิ่นปากนั้นค่อนข้างสำคัญกับชีวิตประจำวันของปูเป้ เนื่องจากเราต้องใกล้ชิดกับคนจำนวนมาก โดยเฉพาะวันที่ไปออกงานหรือวันที่ต้องสอน Workshop นั้น เราต้องมั่นใจและต้องกล้าพูดเป็นพิเศษ ซึ่งนอกจากเหนือจากการแปรงฟันและการทำความสะอาดลิ้นแล้ว อีกหนึ่งตัวช่วยที่ปูเป้ขาดไม่ได้เลยก็คือ “น้ำยาบ้วนปาก” ซึ่งทางแบรนด์ Listerine เห็นว่าปูเป้และครอบครัวก็ใช้น้ำยาบ้วนปากของเขาอยู่ประจำมาเป็นสิบปี จึงอยากให้ปูเป้มาแชร์ประสบการณ์และมุมมองและประโยชน์ในการใช้น้ำยาบ้วนปาก จึงเป็นที่มาของบทความในวันนี้

ยกตัวอย่างในกรณีของฟันนั้น ผิวชั้นนอกที่เป็นของเแข็งของฟันเรียกว่า Enamel อันประกอบไปด้วย Hydroxyapatite อันเป็นสารประกอบของแคลเซียมและฟอสเฟต ถึง 98% เหนือชั้น Enamel ของฟันจะมีเยื่อเมือกที่สร้างจากไกลโคโปรตีนของน้ำลายเคลือบไว้เป็นชั้นบาง ๆ เรียกว่าชั้น Perlicle ซึ่งชั้นเยื่อเมือกเคลือบฟันนี้สามารถถูกขัดออกได้ง่ายด้วยการบ้วนปากหรือแปรงฟัน แต่ก็จะถูกเติมใหม่ทันทีเมื่อฟันสัมผัสกับน้ำลายแม้หลังแปรงฟันเสร็จใหม่ ๆ โดยอัตโนมัติ

ชั้น Pellicle นี้ตอนเริ่มสร้างจะยังไม่มีเชื้อแบคทีเรียมาเกาะ ทำหน้าที่เป็นตัวปกป้องผิวฟันกันพวกกรดจากอาหารที่กินเข้าไป เมื่อเวลาผ่านไปจะหนาตัวขึ้นเป็น Biofilm แผ่นฟิลม์นี้เป็นตัวที่ทำให้แบคทีเรียสามารถมายึดติดได้ ก่อปัญหาสุขภาพเหงือกและฟัน ทำให้เกิดฟันผุเมื่อแบคทีเรียที่อาศัยอยู่ใน Biofilm ทำการกินสารกลุ่มแป้งและน้ำตาลแล้วผลิตของเสียเป็นกรดเพื่อทำลายผิวฟัน (Deminerlization) บนชั้น Enamel ซึ่งก่อให้เกิดคราบ Plaque (พลัค) และคราบ Plaque นี้ยังก่อทำให้เกิดปัญหาเหงือกร่น เหงือกอักเสบได้อีกด้วย

โดยปกติแล้วคราบ Plaque จะมีสภาวะเป็นกรดจากการที่แบคทีเรียผลิตของเสียเป็นกรดออกมา แต่เมื่อคราบ Plaque อยู่มานานขึ้น อาหารเริ่มหมดลง และแบคทีเรียเริ่มตาย ค่า pH ของตัว Plaque จะสูงขึ้นและอยู่สภาวะที่เหมาะกับการสะสมของแร่ธาตุแคลเซียมจนกลายเป็นหินปูนแข็ง ๆ ได้ในที่สุด ซึ่งหินปูนนี้ไม่สามารถกำจัดออกได้โดยวิธีบ้วนปากและแปรงฟันอีกแล้ว จำเป็นต้องพบทันตแพทย์เพื่อทำการขูดออก

จะเห็นได้ว่าเจ้า Biofilm ที่แบคทีเรียผลิตเพื่อเป็นแหล่งอาศัยของพวกมันเองนั้นไม่ได้ก่อให้เกิดแค่เรื่องของกลิ่นปาก แต่ยังนำมาถึงปัญหาสุขภาพช่องปาก ฟัน และเหงือกอีกด้วย ขั้นตอนการเกิด Dental plaque นั้น จะต้องใช้เวลานานพอสมควร กว่าเชื้อจุลินทรีย์จะมาเกาะที่ผิวฟันจนสามารถปล่อยสารพิษที่ทำให้เกิดโรคได้ ถ้านับเวลาหลังจากทำความสะอาดฟันจนถึงเกิด Dental plaque แล้วจะใช้เวลาประมาณ 24 ชั่วโมง และจะต้องสะสมมากพอที่จะปล่อยสารพิษมาทำลายผิวฟันและเหงือกต้องใช้เวลาถึง 48 ชั่วโมง นี่แสดงให้เห็นว่าปัจจัยทั่วไปที่ก่อให้เกิดกลิ่นปาก ฟันผุ และปัญหาเรื่องเหงือกนั้นสามารถดูแลได้ด้วยการรักษาสุขอนามัยของช่องปากที่ดีด้วยการแปรงฟัน แปรงลิ้นร่วมกับใช้ไหมขัดฟัน วันละ 2 ครั้ง และบ้วนปากทุกครั้งหลังมื้ออาหาร

แต่ถึงแม้เราจะทำทั้งหมดนี้แล้ว เราก็ยังไม่สามารถจัดการกับ Biofilm และแบคทีเรียในส่วนที่การใช้แปรงเข้าไม่ถึง การใช้น้ำยาบ้วนปากจึงเป็นตัวช่วยที่ทำให้การดูแลสุขอนามัยในช่องปากได้สมบูรณ์ยิ่งขึ้น

 ในน้ำยาบ้วนปากนั้นจะมีส่วนผสมของสารที่ช่วยขจัดแบคทีเรียอยู่หลายกลุ่ม ถ้าเป็นยาฆ่าเชื้อ ก็ได้แก่พวก Chlorhexidine ซึ่งแพทย์จะสั่งให้ใช้ตามแต่กรณีไป ถ้าเป็นสารที่ใช้ในยาบ้วนปากทั่วไปได้ก็อย่างเช่น Tricolsan และอีกตัวที่นิยมใช้ก็คือสารกลุ่ม Essential Oil   

ประสิทธิภาพในการลดแบคทีเรียของสารเหล่านี้ขึ้นอยู่กับความสามรถในการแทรกผ่าน Biofilm อีกด้วย ซึ่ง Biofilm ที่จัดการได้ง่ายคือฟิลม์ที่พึ่งเกิดขึ้นใหม่ ๆ การปล่อยปละละเลยเรื่องความสะอาดในช่องปากทำให้เกิดการสะสมของ Biofilm ที่มีระเบียบและหนาขึ้นส่งผลให้การแทรกผ่านและการขจัดไปก็ยิ่งทำได้ยากขึ้น  

มีการศึกษาพบว่า Essential Oil จากธรรมชาติมีฤทธิ์ระงับการเจริญเติบโตของเชื้อจุลินทรีย์ นอกจากนี้ยังมีคุณสมบัติที่ดีมากในการแทรกผ่านเข้าไปในชั้น Biofilm ที่มีการสะสมตัวมาเป็นระยะหนึ่งได้ดีกว่าสารกลุ่มอื่นอย่าง Tricolsan และก็ไม่ก่อให้เกิดการต้านทานของเชื้อแบคทีเรียอีกด้วย นอกจากนี้ Essential Oil ยังทำให้น้ำยาบ้วนปากมีรสชาติดี และทำให้กลิ่นปากหอมสดชื่นขึ้น

สิ่งที่สำคัญในการที่น้ำยาบ้วนปากที่ผสมสารยับยั้งเชื้อแบคทีเรียเหล่านี้จะทำงานได้ผลดีนั้นขึ้นอยู่กับระยะเวลาในการสัมผัส นั่นหมายความว่าถ้าเขาให้อมกลั้วปาก 30 -60 วินาทีก็ต้องอมกลั้วปากไว้ 30 - 60 วินาที ไม่น้อยไปกว่านั้น
